Leech Therapy (Irsal-e-Alaq): Nature’s Bio-Surgery

Unlocking the "Living Pharmacy" for Tissue Repair

Dr

Consultant Unani Physician | MD (General Medicine)

1Introduction

Irsal-e-Alaq (Leech Therapy) is a specialized method of evacuation in Unani medicine used to remove morbid matter from deep tissues and localized areas of congestion. While often misunderstood as a primitive practice, modern science now classifies the medicinal leech (Hirudo medicinalis) as a "Living Bio-Pharmacy" due to the complex cocktail of bioactive enzymes found in its saliva. At Al Hashmi Health Centre, we utilize this therapy not just for blood removal, but as a precise method to inject these natural healing compounds directly into the bloodstream of the affected area.

2The Unani Perspective: Resolvent & Evacuation

In Classical Unani texts (referenced by Ibn Sina and Al-Zahrawi), Leech Therapy is preferred over Venesection (Fasd) for treating skin diseases and localized inflammation.

Mechanism of Action in Unani:

Tanqiya-e-Mowzi (Localized Purification)While Fasd cleanses the general circulation, leeches are used to suck out "sedimented" or stubborn morbid humors (Akhlat-e-Ghaleeza) trapped in the skin and micro-vessels.
Tahleel (Resolution)The therapy dissolves hard swellings and nodules by breaking down the solidified matter causing the blockage.
Targeted ActionIt is the treatment of choice for areas where veins are too small to slit, such as the face, gums, or fingertips.

3The Modern Scientific Perspective: The Enzyme Cocktail

Modern research confirms that the therapeutic benefit of Leech Therapy comes primarily from the saliva injected during the feeding process. This saliva contains over 100 bioactive peptides and proteins that act as powerful therapeutic agents.

Hirudin (The Super-Anticoagulant)

Prevents blood from clotting locally, ensuring that blood continues to flow through the area even after the leech is removed. This washes out toxins and re-establishes circulation in dead/dying tissues (crucial for Diabetic Foot).

Vasodilators

Widen the blood vessels, immediately increasing blood flow to oxygen-starved tissues, promoting rapid healing of non-healing ulcers and wounds.

Hyaluronidase

Breaks down hyaluronic acid, increasing tissue permeability and allowing healing enzymes to penetrate deeper into the infection or wound.

Bdellins and Eglins

Powerful anti-inflammatory agents that significantly reduce swelling, redness, and pain in conditions like Arthritis and Varicose Veins.

Natural Anesthetics

Leeches release a mild anesthetic bite, ensuring the procedure is largely painless for the patient.

Clinical Indications at Al Hashmi Health Centre

Non-Healing Diabetic Ulcers: To restore micro-circulation and prevent gangrene/amputation.
Varicose Veins: To dissolve the pooled blood and reduce venous pressure.
Alopecia (Baldness): To stimulate blood flow to dormant hair follicles.
Chronic Eczema & Psoriasis: To remove localized toxins and reduce plaque thickness.
Reconstructive Support: Improving blood flow in reattached tissues or grafts.

Precision & Care in Every Session

Our therapy sessions are conducted in a completely sterile, clinical environment. Each placement is calculated based on the specific condition and the underlying vascular anatomy, ensuring maximum therapeutic benefit with minimal discomfort.

Single-use Sterile Leeches Only

Safety & Hygiene Protocol

To address modern concerns about infection control:

Single-Use Policy

At Al Hashmi Health Centre, we never re-use a leech. Once used, it is disposed of as per biomedical waste protocols to eliminate any risk of blood-borne disease transmission.

Source

We use only medically bred leeches, not wild leeches, to ensure they are free from environmental parasites.

Medical Disclaimer & Contraindications

    Bleeding Disorders (Hemophilia, Warfarin/Heparin users)
    Severe Anemia (< 10 g/dL)
    Immunocompromised Patients (Chemotherapy, HIV/AIDS)
    Pregnancy
    Keloid Tendency

Note: Post-procedure bleeding is normal and expected in Leech Therapy. It can last for 4 to 12 hours. This is part of the therapeutic process. We provide specialized pressure dressings to manage this safely.
