In Unani terms chronic sinusitis is the accumulation of cold, viscid matter in the cavities of the head, sustained by a weak digestive faculty and repeated catarrh. Treatment thins and evacuates that matter, warms and strengthens the affected passages, and then works on the tendency to re-accumulate, which is what separates treatment from another course of decongestants.

3. What treatment involves

  1. Examination, and review of any CT or endoscopy report and previous antibiotic courses
  2. Formulations to thin and clear the accumulated matter, with steam and nasal preparations
  3. Hijama at head and neck points where the assessment indicates congestion
  4. Digestive and dietary correction to reduce recurrence, including cold and mucus-forming foods
  5. Review with attention to polyps or structural blockage, and ENT referral where surgery is genuinely indicated

I have had four courses of antibiotics. Why does it keep coming back?

Because antibiotics clear an infection but do not change what allows it to recur — the retained matter, the tendency to catarrh, and in some patients a structural blockage or polyp. The Unani protocol addresses the first two directly and looks for the third rather than assuming it is absent, which is why an existing CT report is worth bringing.

Will I need sinus surgery?

Most chronic sinusitis is managed without an operation. Where there are large polyps, a markedly deviated septum or anatomy that will not drain whatever is prescribed, surgery may genuinely be the right answer and we will refer you rather than keep treating around it.

Does steam inhalation really help?

It helps, and it helps more when it is done correctly and with the preparation you are given rather than plain water once a week. You will be shown the frequency, duration and posture, because done badly it is ineffective and done carelessly it scalds.
