Treatment also addresses the gut, since digestive function and thyroid function influence each other closely.

Will I be able to come off thyroxine?
It depends on why the gland is underactive and how much function remains. Patients with subclinical or mild hypothyroidism sometimes reach normal TSH levels and reduce or stop medication under supervision; those whose gland has largely failed, or who have had it removed, will need replacement for life and should be told so clearly. Any change is made on repeat blood tests, not on how well you feel.
Why does the treatment differ for hypo and hyper thyroid?
Because in Unani terms they are opposite derangements. An underactive gland is treated with warming, stimulating formulations; an overactive one with cooling, sedating preparations. Giving either the other's medicine makes it worse, which is the reason a current thyroid profile is required before anything is prescribed.
Does the neck swelling go down?
A diffuse goitre associated with disturbed function often reduces as the gland's function is corrected. A discrete nodule is a different matter and is investigated rather than treated blind — ultrasound first, and referral if the features warrant it.
