2. How Unani medicine understands it

Unani medicine calls vitiligo Bars and understands it as a derangement in which the skin loses its capacity to form its natural colour, associated with morbid humor and impaired hepatic function. Treatment combines internal correction with photosensitising topical preparations and controlled sun exposure, so that pigment returning at the edges and centre of a patch is the marker of progress.

3. What treatment involves

  1. Documentation of every patch with dated photographs, so progress is measured rather than remembered
  2. Internal formulations to correct the underlying derangement and support liver function
  3. Topical photosensitising preparations with a precise, supervised sun-exposure schedule
  4. Fasd (venesection) or leech therapy in selected cases, where the assessment indicates it
  5. Monthly review against the photographs, with the protocol adjusted on what the images show

How long before pigment starts coming back?

Vitiligo is slow, and being honest about that is part of the treatment. Early repigmentation — small dark dots inside a patch or colour creeping in from the edge — is usually seen after two to four months of consistent treatment, and a full course runs considerably longer. Patches on the face and trunk respond better than those on the fingertips, lips and bony areas, where response is limited in every system of medicine.

Is vitiligo contagious, and will my children get it?

It is not contagious in any way — it does not spread by touch, sharing food, or living together. There is a familial tendency, so a child of a parent with vitiligo has a somewhat higher chance than average, but most children of affected parents never develop it. The social beliefs attached to white patches in India cause more harm than the condition, and we will discuss that plainly with you.

Do I have to sit in the sun?

Controlled exposure is part of the protocol when photosensitising applications are prescribed, and the dose matters: too little does nothing, too much burns the depigmented skin badly because it has no protection. You will be given exact timings for your skin and the season, and told when to stop.
